The enrollment form collects a name, phone number, email, selected group, optional comment, and whether a supported place was requested. We use this data only to contact you about enrollment.
After submission, data is stored in a protected application-record system. Only an authenticated operator with the required permission can access the request for 90 days. The request is physically deleted automatically after that period once any related benefit document has been safely destroyed.
